The T4, Total (Total Thyroxine) test is a blood test used to measure the levels of thyroxine hormone in the body. Thyroxine is produced by the thyroid gland and is essential for regulating metabolism and energy levels. This test helps diagnose thyroid conditions such as hyperthyroidism or hypothyroidism, monitor thyroid hormone replacement therapy, and evaluate overall thyroid function. It provides valuable information to healthcare professionals in determining the cause of various symptoms related to thyroid dysfunction and guiding appropriate treatment plans.

T4, or thyroxine, is the primary thyroid hormone produced by the thyroid gland. While most T4 is bound to proteins, a small fraction remains unbound or free. Free T4 testing evaluates the circulating T4 that is accessible to body tissues and can exert its effects. Total T4 testing measures the overall amount of the hormone, encompassing both free T4 and the portion bound to proteins.
A T4 test may be requested by a doctor for various reasons, commonly as a follow-up to an abnormal TSH test result. It is also employed when there are symptoms suggestive of a thyroid disorder. Conditions like hypothyroidism and hyperthyroidism are prevalent, and if symptoms indicate these disorders, a doctor may order a T4 test along with other thyroid function tests, such as TSH and T3, forming a comprehensive thyroid panel.
Typically, no specific preparation is needed for a T4 test. Your doctor will inform you if there's a requirement to stop taking any medications or supplements before the test. In some cases, discontinuing Biotin (B7) supplements briefly may be necessary to avoid interference with the test results.
